Grant funds expansion of ‘medical homes’ biz model

Five months after receiving a grant to retool the way Oregonians receive primary health care, two providers are jointly working to convert several medical clinics to a model they hope will improve care, save money and attract more physicians.

The grant providers hope the team-oriented “medical homes” model will lower health care costs by keeping patients healthier and by making primary care more satisfying for physicians. The model spreads responsibility for patient care across a team that includes nurses, dentists, psychologists and other professionals.
